Welcome to the surgery feedback registration survey.
In this survey we would like you to provide some basic details about yourself. This just enables us to know who you are and attribute your comments to the right surgery. After registration there will be an online forum followed by a more detailed questionnaire in the coming weeks. You can take part in either or both.
We are conducting this feedback process in partnership with Tpoll, a Market research Agency and member of the Market Research Society. Tpoll therefore abides by their code of conduct and the Data Protection Act 2008 and as a result your personal details will only be used for this feedback project and not passed on to any third party. Also you will not be identified personally to the surgery as your comments will be passed back in aggregate form so please be as honest as you can.
