We are keen to hear patients' views about how the practice is doing. We would like to be able to find out the opinions of as many patients as possible and are asking that you give your view via this short survey.
We have asked Tpoll, an Independent Market research Agency and a member of the Market Research Society, to conduct the survey on our behalf. Tpoll abides by the MRS code of conduct and the Data Protection Act 2008 and as a result your personal details will only be used for this feedback project and not passed on to any third party. Also you will not be identified personally to the surgery as your comments will be passed back in aggregate form so please be as honest as you can.
